Children’s Heart Surgery Fund works to ensure that all families who stay in the CHSF accommodation on Brotherton Wing are able to live in a safe and healthy environment.
Your residence in CHSF accommodation depends on your collaboration with CHSF and respect for others.
We therefore ask all parents and carers using the CHSF accommodation to read and agree to the following guidelines, and promote the safety of all those using the service.
Once you have read the guidelines complete and submit the form below.
- Please read and observe the guidance contained in the booklet in your room which offers information around health and safety, as well as useful information and cleaning procedures.
- Do not welcome visitors in the CHSF accommodation, only family members who are residing within the rooms are permitted to use the facilities. (Details of those who are able to stay must be recorded on the form below).
- CHSF have the right to terminate your stay if any persons other than those recorded here are found to be occupying the room. This will result in you having to vacate the CHSF accommodation immediately.
- If you notice anything in the CHSF accommodation which may cause injury or accident, please inform a member of CHSF staff, or Ward L51 staff as soon as possible.
- Please turn appliances off when not in use to save energy bills.
- Please respect the need for cleanliness. Guests are responsible for cleaning up after themselves, this includes the Laundry Room and the Kitchen.
- Smoking or vaping is not permitted in the CHSF accommodation.
- No alcohol is permitted in the CHSF accommodation.
- No drugs or use of drugs is permitted in the CHSF accommodation. Failure to comply will result in eviction from the property and incidents may be reported to the Police.
- CHSF staff may need to enter rooms occasionally, such as when maintenance is required, or for weekly room inspections.
- Noise levels must be kept to a minimum, especially after 9pm.
- Laundry must be removed from the washer and dryer promptly.
- Aggressive, threatening and abusive behaviour will not be tolerated and will result in immediate removal from the CHSF accommodation.
- Please ensure all key cards for rooms are handed in to Ward L51 upon departure.
- In the event of an emergency or safeguarding concern, CHSF Family Support team and Hospital Security are permitted to enter the room.
- Due to room capacity one child under the age of 5 years is permitted to stay with parents in the accommodation where alternative arrangements cannot be made. We ask that children use an age-appropriate travel cot which can be provided by CHSF upon request.
- Please note that your child is your responsibility during your stay in the accommodation and must be supervised at all times, under no circumstances should a child be left in the room unattended. Failure to comply may result in a cause for concern and the LTHT Safeguarding team will be notified in line with policies and procedures.
