Helping you feel confident about what’s next.
Moving from children’s to adult heart care can feel like a big step, but you don’t have to figure it all out on your own. Our Transition Booklet (sometimes called a “Transition Passport”) is here to help you get prepared, feel more confident and stay in control of your journey.
The Transition booklet has been created with both our young people and clinicians at the Leeds Congenital Heart Unit, so it’s designed to be genuinely useful, easy to understand and relevant to you.
It’s your space to start thinking about what matters to you and what the next stage might look like.
Inside, you’ll find sections to help you explore things like your future goals, your condition, medication and exercise, as well as plenty of space to write things down. You can use it to jot notes, track important information and write down any questions you want to ask your care team.
It’s also a great way to get ready for your transition phone call or appointments, so you can feel more confident speaking up and making decisions about your health.
Transition Evenings
Transition Evenings are held virtually online by the Leeds Congenital Heart Unit, providing essential information twice a year. You will also receive a ‘Welcome To The Adult Service’ telephone appointment prior to your first ACHD appointment, once you have been referred to the Adult service.
Many young people receive their booklet ahead of their transition appointment, but if you haven’t had one yet – or would like another copy – just speak to your cardiac nurse team or CHSF’s Family & Youth Support Worker Lucy.
This is your heart journey, and the Transition Booklet is there to help you take those next steps at your own pace.

This guidebook, created by CHSF and the team at LCHU, is for teens aged 12-18 who are coming in for heart surgery at Leeds General Infirmary and living with a congenital heart defect. Request a copy from your Cardiac Nurse Specialist or email the team using the link below.
